A duty pharmacy is one that has been designated to open during normally non-working hours, such as Sundays and holidays. Two types of duty service should be distinguished:
Open shutter duty: it's open all day from 8am to 9pm;
Closed shutter duty: it's closed, but the pharmacist is at home and can open and make themselves available at any time.
Thanks to this emergency service, you can purchase prescription medications and other health products outside the regular opening hours. The duty service coupled with the emergency service allows anyone in need to obtain medications 24/7 throughout France. If you have just consulted a doctor, they will indicate the nearest duty pharmacy. You can also go to your usual chemist shop which, in principle, displays all duty schedules in its window, or consult the local press which generally reports all duty services, if you live in the provinces.

There are about ten chemist store open at night 24/7 in Paris, others until 9 PM. Given the increasing number of Group A streptococcus infections, the General Directorate of Health (DGS) urges people suffering from sore throat to take a diagnostic test of the throat.
There are 2 types of sore throat: viral sore throats caused by a virus, and bacterial sore throats caused by bacteria. Viral sore throats are more common and do not require antibiotics. Bacterial sore throat, which can be caused by bacteria such as Group A streptococcus, requires antibiotic treatment.
The testing process has been simplified, and pharmacists can now perform it directly in the chemist shop for a patient, without a prescription:
for children 10 years and older;
for adults when the symptoms presented justify performing the test.
The TROD test (Rapid Diagnostic Orientation Test) or strep test consists of a painless swab taken in a few minutes from the tonsils:
If the test is positive, the pharmacist can dispense antibiotics. Given the current context, the DGS recommends amoxicillin as the first-line treatment and self-isolation for 48 hours.
However, if the test is negative, antibiotics are unnecessary because the sore throat is caused by a virus. Antibiotics could promote the emergence of antibiotic-resistant bacteria. Pharmacists offer patients treatments that relieve pain, such as paracetamol and throat lozenges.
The test strip changes color in just 5 to 10 minutes. After this time, the healthcare professional can read the test result.
The management of sore throat is remunerated according to two rates, depending on the situation, via the billing of the "TRD" code: €10.50 or €15.70 including tax in the DROMs (French overseas departments).
When the patient presents spontaneously and the pharmacist does not dispense antibiotics after the test.
Or when the patient is referred by their doctor with a conditional prescription for antibiotics.
The Social Security Financing Act for 2024 expands pharmacists' prerogatives, now allowing them to perform a urinary test in the pharmacy, without a prescription, in cases of suspected cystitis.
This test is intended for women aged 16 to 65, under certain conditions: it is not accessible to pregnant women, those at risk of pregnancy, those who have had a urinary infection in the last 15 days, or those who have experienced more than three urinary infections over the past 12 months.
The pharmacist uses a urinary test strip to confirm or rule out the presence of an infection. If the test is positive, the patient will be directed to her doctor for a possible prescription. In case of a negative test, the pharmacist excludes the diagnosis of urinary infection and recommends a medical consultation. The result is obtained in a few minutes and is systematically communicated to the patient's primary care physician.
Pharmacists now have the ability to prescribe and administer vaccines to adults and children over 11 years old, for those included in the vaccination schedule (such as those against diphtheria, tetanus, polio, whooping cough, flu, human papillomavirus, measles, mumps, rubella, and COVID-19), while respecting the associated recommendations. This expansion of skills, however, excludes the prescription of live vaccines for immunocompromised people.
Before November 2022, pharmacists could already administer 15 vaccines from age 16, but a medical prescription was required. Now, they can not only perform these injections but also prescribe them, thus eliminating the need to consult a doctor for this purpose.
The shared medication review is intended for patients over 65 years old with chronic conditions who have at least five medications prescribed for a duration of at least six months. This review aims to answer patients' questions about their treatments, enhance their understanding and adherence, as well as optimize the management of their medications. This approach consists of evaluating the treatment considering comorbidities, possible geriatric syndromes, patient preferences, and using tools to identify potentially inappropriate medications.
The Preparation of Doses to be Administered (PDA) consists of organizing in advance, for a defined period (usually 7 days), the doses of medication to be taken in a personalized manner and in accordance with the medical prescription. This practice aims to improve treatment management and adherence by ensuring rigorous monitoring of doses and reinforcing the safety and traceability of medication administration.
It is estimated that only half of patients with chronic conditions correctly follow their treatment, which can lead to serious consequences, including reduced therapeutic efficacy and the occurrence of adverse effects.
This adherence issue is also present in the treatment of acute illnesses. Improper use of medication affects patients’ quality of life and life expectancy while generating additional costs for our social protection system. Elderly individuals and those with certain disabilities are most at risk.

Pharmacies and parapharmacies do not provide the same selection of products. In fact, parapharmacies focus on selling French cosmetics care items (hygiene, beauty...) that are available over-the-counter, while pharmacies also provide the sale and dispensing of both prescription and non-prescription medications. This significant difference is explained particularly by French legislation which does not allow parapharmacies to sell medications, even those without prescription (self-medication health items).

A community pharmacy must have a doctor of pharmacy at its head. Depending on the size of the establishment, they are assisted by associate pharmacists, as well as pharmacy technicians. The presence of at least one pharmacist is required from the opening to the closing of the pharmacy. Moreover, even if the other professionals who work alongside the pharmacist are not considered healthcare professionals, they still receive advanced training in the paramedical sector. Even though it is not a legal obligation for parapharmacies, some brands require that a pharmacist be present and available in the store to provide advice and explain how certain products work on the body and cells. For the sell of products, especially those prescribed on prescription, patients will benefit from coverage of their health purchases by Health Insurance. The remainder of the bill can, if necessary, be reimbursed by the patient's supplementary health insurance. However, in parapharmacies, all items must be paid for by the customer. Afterward, the customer can request coverage by their supplementary health insurance depending on their contract.

Since most prescribed medications are reimbursed by social security, their prices are set by Health Insurance according to specific criteria. The pharmacist owner therefore has no flexibility regarding the selling price of medications.
